Due to the nature of the exercises undertaken by the students of a dance school, parents are warned that children suffering from asthma, orthopedic injuries or degenerative back complaints should seek advice from an appropriate medical practitioner, or specialist, before enrolling a student in the dance school. All reasonable care will be taken for the welfare of the students; however, Excelsior School of Dance will assume no responsibility in the case of injury or accident to any student. In the event of a medical emergency and where a parent cannot be contacted, I authorize Excelsior School of Dance to seek medical advice and treatment.

This agreement fully represents all terms and considerations and no other statements, conditions, or promises are made in conjunction with this release form. I give permission to Excelsior School of Dance to use, without charge and without reservation, my likeness in any medium and for any lawful purpose, including promoting the institution, its programs and services. Studio Policies:

Please review and agree to our studio policies:

  1. CommunicationEMAIL is our main means of communication.  Please check your email regularly for class changes or last minute class updates.  The best way to communicate with teachers is by emailing the studio at excelsiorschoolofdance@gmail.com. Phone messages may not be returned the same day as office hours are limited. Teachers cannot accept calls during class time.
  2. Parking Lot:
    • PLEASE DO NOT EVER LET YOUR CHILD PLAY IN THE PARKING LOT. We often see parents chatting in the lobby after class while their child walks out the door without them. We also see parents chatting outside the studio while their child plays between cars or in the neighboring business parking. This is absolutely against studio policy. Children are not to be left outside without supervision or leave the building without an adult.
    • Where to park- We have designated parking area; please do not park in "Director Parking" or "Drop off Area"
    • In order for children to play outside they must be accompanied by their parent
  3. Viewing window: The viewing window gives you an opportunity to watch class every so often and see the student's progress. Please feel free to watch class. If you notice students paying more attention to you than the instructor, please move away from the window. You might be a distraction to other students in the class, or possibly the teacher. Do not allow siblings to stand in lobby chairs while watching through the viewing window. Please make sure siblings are not distracting class.
  4. The lobby: Families are always welcome in the lobby but please keep in mind that lobby space is limited and it gets really hot in the waiting area. If you utilize the lobby, please throw trash away and straighten toys before leaving. Please replace chairs if you move them.
  5. Before and after class: Most classes start as a class ends. Please gather your child and be prepared to leave the building after class is over. Please be courteous to the class that comes after your class is over.
  6. To Change or Drop Class: In order to change or drop a class you must fill out a "Change Request Form"  These can be found in the office in the "Forms Box" or on the website www.excelsiordanceschool.com
  7. Production: As students of Excelsior School of Dance & Performing Arts, each dancer has the unique opportunity to perform in a full ballet production at the end of their dance year with Ballet Excelsior of Houston, (our very own professional dance company)! The entire dance school participates in sharing a message of hope, while demonstrating the dance skills they have learned and practiced throughout the year.
  8. Ballet EXAMS: At Excelsior School of Dance we use a syllabus in which the students memorize and able to concentrate on their technique then are examined by a certified examiner from the organization.
  9. Water: Please bring bottled water to each class.  If you forget, we sell bottled water in the foyer for $1.00 each, but please keep in mind that this is not always in stalk.
  10. Attire: Please bring your student(s) to the studio prepared for class- Have the student(s) dressed in the necessary attire:
